The Portable Wellness Clinic program in Pomona was established in 2003 as a demonstration program to provide cost-effective health care access. Its mission is to help individuals who lack the resources to get medical attention for their health problems.
The service is offered to anyone, independent of where they live, whether they have insurance, or have an ID.
This website has been set up for you. You might be a family member of someone who can benefit from seeing a doctor at the Portable Wellness Clinic. Perhaps you work at a social service agency or organization that helps those in transition. Please use the information on this website to stay current.
The Portable Wellness Clinic in Pomona is a joint community service
program brought to you by the Family Health Center (affiliated with
Pomona Valley Hospital Medical Center) and Compassion Net Inc..
It is based on the model developed by Compassion Net, which spearheaded
the first Portable Wellness Clinic in San Bernardino in 1998, in
collaboration with San Bernardino's county hospital and clinics.
The Lipid Profile / Cholesterol test is temporarily
NOT AVAILABLE . Please call (909)920-9876 for
